New Patient Information
Prior to your first appointment, you’ll need to fill out our new patient forms. We use this information to make decisions about your treatment and get to know you a little better, so they’re very important to your overall results and experience. Out of respect for your time, we’ve provided the forms here for you to download, print, and fill out prior to your first appointment. Bring them with you, and your check-in process will go smoothly

- Consent for Release of X-Rays
(this may be required if you would like us to obtain your x-rays from another office)
- Authorization for Minors
(this form is required if someone other than the parent/legal guardian will bring a minor to their appointment)
- Pre-Op Sedation Instructions
(read and sign if you will receive sedation for your dental procedure)
- Post-Op Oral Surgery Instructions
General Appointment Information and Office Hours
Monday-Thursday 8:30-12:30, 1:30-5:00
The office is closed on weekends and major holidays. Our answering machine will always be available to take messages and provide emergency information. We make every effort to honor time commitments and request that you extend the same courtesy to us. If you cannot keep an appointment, please notify us at least 24 hours in advance of your scheduled appointment time. This courtesy makes it possible to give your appointment to another patient who desires to see the doctor or hygienist. A fee will be charged for cancellations and appointment failures with less than 24 hours notice.
What to Bring to Your Appointment
Whether you are seeing the doctor or the hygienist, we will review and update your medical history at each appointment. Please bring a list of any medications you take with you to your appointment.
Also, please bring contact information for your general medical doctor and any specialists you see such as a cardiologist, orthopedist, or oncologist. Bring your current dental insurance card and your driver's license.
Fees and Payment
Your payment is due at the time services are rendered. Please be prepared to pay at each appointment. If you need financing, you may qualify for CareCredit. Our administrative staff would be happy to assist you in learning more about this option.
